I recommend you call the customer service number on the back of your insurance card to confirm benefits and eligibility. Some helpful questions to ask:

• What is my in-network coverage for mental health services? Do I have a deductible and/or coinsurance?

• Does my plan cover CPT codes 90791, 90837, and 90834 (the codes I use for our sessions)?
  

If you have a different insurance carrier, I can provide you with a receipt for services (called a Superbill) that you can submit to your insurance company for reimbursement.

Many health insurance companies provide out of network benefits for mental health services. Please call the phone number on the back of your card to learn more about your out-of-network services. Some questions to ask:

• Do I have out of network coverage for mental health services?

• What is my out of network deductible?

• Does my plan cover CPT codes 90791, 90837, and 90834 without pre-authorization?
  

Self Pay

If you plan to pay for treatment on your own, you will be charged my rate of $250 for the initial intake appointment, and $200 for ongoing weekly sessions.

All payments are due at the time of service, including deductibles, co-payment or co-insurance, and self-pay fees.
